Assistance with the completion of forms
A member of your family or your local asbestos support group may be able to provide assistance with completing the Postal Questionnaire. You may also contact the AMR Information Line for assistance.
AMR Information Line
The AMR Information Line is a toll-free telephone helpline. The AMR Information Line provides general information about the AMR, assistance with completing AMR forms and supports patients who self-notify to the AMR about their diagnosis.
